Wisconsin's Medicaid Family Planning Program ("Waiver") allows basic reproductive health care services to qualified women ages 15 - 44 if they are at or below 200% of the poverty level and not receiving other services through medical assistance. Teens are eligible based on their income, rather than their family's income.

It's EASY to register! Just call the Wisconsin Family Planning and Reproductive Health Association at 1-800-246-5743. Also, check out www.FPHS.org.

Services covered under the program include:
  • Routine reproductive health exams and tests;
  • Contraceptive supplies
  • Pregnancy testing and counseling;
  • Emergency contraception;
  • Diagnosis, testing and treatment of sexually transmitted infections.

Any Medicaid-certified provider can offer services under the Family Planning Waiver:

  • Family planning clinics;
  • Health departments;
  • Physicians;
  • Nurse practitioners;
  • Federally qualified health centers.

Register and receive services on the same day! A process called "presumptive eligibility" provides temporary coverage under the waiver. Women can apply to the waiver program at the clinic where they go to receive services, and then receive services the same day they apply. Application is then made for enrollment that lasts a year.
